CONTRACT TITLE INTENDED PURPOSE

Agreement for Services (AFS) - REVISED 04/2024

This Contract covers various types of services, including but not limited to consulting, technology, engineering, architectural, legal, medical, training, interpreting, and other professional services. Examples of such services include hiring a consultant for business strategy advice, engaging a software development company for a new project, contracting an an attorney for legal advice, or employing a medical expert for specialized training. (do not use for Public Works)

Independent Contractor Agreement (ICA) - REVISED 04/2024

This Contract is designed for services provided by an individual or sole proprietor in cases where the scope and complexity are limited. Examples of such services include small equipment repairs, moving services, or photography. (do not use for Public Works)

Limited Engagement Agreement (LEA) -REVISED 04/2024

This Contract is used for speakers, performing arts, workshops, or other limited engagements that are non-recurring and short in duration. (i.e. a few hours to a day or two)

This Contract is used for Public Works such as construction, repairs, or maintenance on District facilities less than $60k in accordance with CUPCCAA. Contract amounts greater than $25k require Exhibit B for Payment and Performance Bonds.

Public Works projects greater than $60k require a competive bid process. Please contact Purchasing for assistance. 

Design Professional Agreement (DPA)

This Contract is used for design services with firms not in the District’s Architectural Pool of prequalified firms.
Master Design Professional Agreement (MDPA) - REVISED 02/2024 This Contract is used for design services with firms that are in the District’s Architectural Pool of prequalified firms.
Project Assignment Amendment (PAA) to Master Design Professional Agreement - NEW 02/2024

This Contract is used in conjunction with the Master Design Professional Agreement to outline specific requirements and services for a project.

Transportation Services Agreeement NEW 04/2024

This Contract is used for bus charter or other transportation services.

Referee Services Agreement for Associations (RSAA)

This Contract is used for sports officiating services such as referees or umpires.

Contract Amendment Form - NEW 04/2024

The Contract Amendment is used to make modifications to an existing agreement, such as altering the scope of the agreement, the amount involved, or the duration of the contract.

FORM TITLE INTENDED PURPOSE
Contract Routing Sheet - REVISED 03/2024

This Form is required to approve Contracts greater than $20,000. Route to the appropiate approvers listed on the Form and include your Contract. After approvals are complete, send via Adobe Sign and indicate Contracts@fhda.edu as the "Acceptor" of the Form. After Board approval or ratification, your Form will be routed to the VC, Business Services for final signature. 

Single Source Justification Over Bid Limit                                                                                                                                         

Purchases of Good or Services exceeding the bid threshold require competitive bidding pursuant to Public Contract Code 20651. Sole/Single source allows for procurement without bidding if no other known source, vendor, or substitute exists. To justify a sole or single source purchase, utilize this form.

Single Source Justification Under Bid Limit

 

Purchases of Good or Services between $10k and the bid threshold requires 3 competitive quotes in accordance with the Purchasing and Payment Authority Grid. To justify a sole or single source purchase where no other known source, vendor, or substitute exists, utilize this form.

BID Project Plan - NEW01/2024                                  

This Form is used to develop a bid calendar in collaboration with Purchasing to document project information for a competitive Public Works Bid (BID).

RFP Project Plan - NEW01/2024

This Form is used to develop a bid calendar in collaboration with Purchasing to document project information for a competitive Request for Proposal (RFP).

Bond Contract Routing Sheet - NEW 03/2024

This Form is required to approve Bond funded Contracts less than the bid limit. Route to the appropiate approvers listed on the Form and include your Contract. After approvals are complete, send via Adobe Sign and indicate Risk@fhda.edu as the "Acceptor" of the Form. After Board ratification, your contract will be returned with the authorized District signature. 

Bond Requisition Request Form - NEW 03/2024

This Form is used to request a requisition for bond funded projects.
