CONTRACT TITLE INTENDED PURPOSE

Agreement for Services (AFS)  UPDATED 4/2025

This Contract is used for a variety of professional services provided by companies or independent contractors. Examples include but are not limited to consulting, software development, legal or medical services, equipment repairs, moving, photography, graphic design, marketing, translation, transcription, web design, training, interpreting, or facilitation services. For other types of services and agreements, please refer to the Purchasing Documents Matrix for Goods and Services to determine the appropriate contract type. (do not use for Public Works). Agreement for Services instructions.

Limited Engagement Agreement (LEA)

This Contract is used for speakers, performing arts, workshops, or other limited engagements that are non-recurring and short in duration. (i.e. a few hours to a day or two)

This Contract is used for Public Works such as construction, repairs, or maintenance on District facilities less than $75k in accordance with CUPCCAA. Contract amounts greater than $25k require Exhibit B for Payment and Performance Bonds. Public Works and Maintenance Agreement instructions.

Public Works projects greater than $75k require a competive bid process. Please contact Purchasing for assistance. 

Master Design Professional Agreement (MDPA) This Contract is used for design services with firms that are in the District’s Architectural Pool of prequalified firms.
Project Assignment Amendment (PAA) to Master Design Professional Agreement

This Contract is used in conjunction with the Master Design Professional Agreement to outline specific requirements and services for a project.

Transportation Services Agreeement

This Contract is used for bus charter or other transportation services.

Sports Officiating Agreement

This Contract is used for sports officiating services such as referees or umpires.

Contract Amendment Form

The Contract Amendment is used to make modifications to an existing agreement, such as altering the scope of the agreement, the amount involved, or the duration of the contract.

 

 

Forms

FORM TITLE INTENDED PURPOSE

No-Cost Contract Routing Sheet 

This form is used to submit MOUs, clinical, internship, revenue-generating, or other no-cost agreements to the District for Board ratification. Include the Contract and relevant supporting documents when submitting this routing sheet.

Request for COI Letter

Utilize this letter template to request a Certificate of Insurance (COI) from your vendor.  Please indicate the date and your email on the letter template.

Worker's Comp Waiver

Vendors who do not have employees are exempt from providing workers’ compensation insurance. To waive this requirement, have your vendor complete and sign this form.

Single Source Justification Over Bid Limit                                                                                                                                         

Purchases of Good or Services exceeding the bid threshold require competitive bidding pursuant to Public Contract Code 20651. Sole/Single source allows for procurement without bidding if no other known source, vendor, or substitute exists. To justify a sole or single source purchase, utilize this form.

Single Source Justification Under Bid Limit

 

Purchases of Good or Services between $10k and the bid threshold requires 3 competitive quotes in accordance with AP 3140. To justify a sole or single source purchase where no other known source, vendor, or substitute exists, utilize this form.

BID Project Plan                                  

This Form is used to develop a bid calendar in collaboration with Purchasing to document project information for a competitive Public Works Bid (BID).

RFP Project Plan 

This Form is used to develop a bid calendar in collaboration with Purchasing to document project information for a competitive Request for Proposal (RFP).

Bond Requisition Request Form 

This Form is used to request a requisition for bond funded projects.
