COVID-19 Information for Vendors/Contractors

Notice To All Vendors Performing Services On-site

  

Dear Colleagues,

The District is committed to protecting the health and well-being of students, faculty, staff, administrators, vendors, and the communities we serve. This is to provide you notice that all vendors, who visit on-site and/or provide in-person services on campus must be fully vaccinated against COVID-19.  This requirement is consistent with the District’s Board Policy 3507 COVID-19 Vaccination requiring all employees working on campus and all students attending in-person classes to be vaccinated, as extended to vendors. 

For purposes of this notification, the term "vendors" includes contractors, independent contractors, and any of its employees or subcontractors.

For Vendors providing services on-site, the Vendor’s employee shall coordinate the following requirements directly with the District Project Manager or the District Representative requesting the service.  The requirements include the following:

  1. Attestation of Vaccination.  The completed form will be kept on file with the appropriate District/Campus Representative and should be carried at all times while on District property.
  2. Health Check Status.  Vendors are required to use the OptimumHQ System to document COVID-19 Health Status and Location Tracking Application, prior to arrival on campus daily or on as-required basis.
  3. Other Safety Protocols.  Vendor shall wear face mask and use safe work practices as recommended by the CDC including social distancing, washing, or sanitizing hands etc.

A contract amendment is not necessary to enforce these requirements and will not be issued. This request is made pursuant to the District’s Standard Instructions and Conditions located at https://purchasing.fhda.edu/forms/index.html  which require your compliance with Federal, State and Local Laws and District Policy.

Any costs incurred to comply with this mandate shall not be passed on to the District either through increases in prices, rates, hours, or any other means. The District reserves all rights at its disposal to enforce the vaccination requirement, which may include, but are not limited to, suspending services, or terminating the contract for breach.

Any questions regarding these requirements should be addressed with your District Project Manager and/or District Representative for whom you are providing the on-site services.
